Do they have negative karma as well? It means someone is keeping an eye on them, likely because they are a spammer.
It's a label so that the user is aware that an admin/mod is watching, also to alert other admin/mods to keep an eye on this user.
We have a 0 tolerance policy for spam and spammers, this is a great example of how the forum manages this.

It's a warning tag for possible spammer users. With this tag moderators can follow them closely.
If they continue, they can get bigger penalties.

When I first saw it, it just seem creepy but I get it now. This is just a simple forum keeping to the simple rule shouldn't be a problem. But then most people think otherwise.
